The JAK Requiem can be unlocked by completing any five challenges from Week 2 of MW3 and Warzone Season 4. These five challenges can be completed across multiplayer, zombies and Warzone.

There are 21 challenges in total for players across the three modes and completing any five of these will reward you with the JAK Requiem Kit.
